Hospice care is provided to individuals with life-limiting illnesses, focusing on comfort, pain management, and support for patients and their families.
Hospice care is suggested for patients with less than 6 months prognosis and supports families coping with changes and loss.
Holistic Home Health Care, Inc. is licensed to provide services throughout the southeastern region of Wisconsin, including Milwaukee, Waukesha, Washington, Walworth, Racine, Kenosha, Ozaukee, Jefferson, Dodge, Sheboygan, and Fond du Lac counties.
Services are available 24 hours a day, 7 days a week, including holidays.
Medicare, Medicaid, Private Pay pays for hospice. Contact a representative for specific pricing.
With client approval, Holistic staff will check your insurance coverage to determine if it will cover your desired services.
After interviews and screening, Holistic conducts multiple background checks and reference checks, then matches caregivers to clients based on needs and personality.
Holistic assembles a team of caregivers for each client. If your usual caregiver is unavailable, an alternate will be assigned and informed of your preferences.
Services begin when hospice clinical and financial eligibility per CMS are met
